How do I create a personalized contact card?

When adding a new contact or when importing a contact I would like to be able
to have my own personalized fields on the card that pops up when I hit the
new contact button. For example, when importing a new contact I would like to
import not only the name, address and company (the basics) but I want to
import specific data (let's say-hair color). How do I add that to all contact
cards?

Note that Outlook doesn't support importing to a custom form. You'd have to
write custom code or use a third-party application. See
