I am designing a contact form with mulitple contact names and accompanying
phone numbers. Do I have to create a new name field (ie phone 1, phone 2,
phone 3 etc.) for each user or can I just use the phone temlpate "business
phone" for each user? If that is the case, when I just want one to merge one
contact name and phone, will the system get confused as to which phone goes
to which name?

You can use the phone number fields in any way you want. It's your data.
