Where can pre punched and perferated paper for printing the outlook small
address book be obtained.

Because Milly helped me , I'll try with this one:  I found six hole
prepunched 3X5 dayrunner pages at Staples, after failing at Office Depot.  
Hard to find in the very corner.  However, I suspect by perforated you are
looking for sheet fed?   I would think that would be an online purchase item.
