I have a database I want to import that has fields that do not correspond to
any fields in Outlook's default list.  I want to create additional custom
fields in outlook and then import my data.

Outlook doesn't support importing to a custom form. You'd have to write
custom code or use a third-party application. See
http://www.outlookcode.com/d/customimport.htm.
