Custom contact form

I added custom fields to my contact form in Outlook, and published it, but I
can't find where it is stored on my computer.  I want to be able to copy it
and share it with some other people - does anyone know where the contact
template for Outlook is stored?

Published forms are stored as hidden items. To create a copy you can give to
someone else, run the form and instead of saving the new item, choose File |
Save As and save it as an .oft template file. Keep a copy as a backup for
yourself.
