How about "Nickname" and "Spouse Nickname"?

An idea, but when I mail merge a letter, I want to use a "salutation" field.
Most contacts will not have a nickname, so Dear "nickname" and "spouse
nickname", won't suffice for all contacts.

What is it you want me to suggest, then?
Outlook  has fields that will accommodate your needs. If you want to create
a custom field, it will not be included in a mail merge.
