I am trying to add new fields on my contact card but am unable to apply those
new fields to ALL the contacts.  How do I apply them to all the contacts and
how to I make those new changes default whenever I need to add a new contact?

You need to publish your custom contact form, then see http://www.outlookcode.com/d/newdefaultform.htm
