How do I import a file to Outlook?

I have followed instructions regarding importing a text delimited file using
the Outlook Wizard.  The mapping fields option is greyed out, and I can't
continue the process.  HELP tells me the following:

The Map Custom Fields button in the Import a File dialog box might not be
available when you try to import data from another program or file. In the
The Following Actions Will Be Performed box, select the Import check box. The
Map Custom Fields button becomes available.

Where do I find the The Following Actions Will Be Performed box?  It's not
one of the screens or questions in my Wizard.

I'm baffled.  Please help.

Is the first line/record in your file a list of field names? I just export a
CSV from Outlook and it uses the first line/record for the field names.
Also, just to be sure, what version of Outlook are you using?
