outlook 2003: custom contact form

hi,
I've got Outlook2003 and use it with Microsoft Exchange. One of my
clients keeps messing up the contact form, i created. For example it
opens the contact with the standart form, or doesn't open it at all, or
opens the old contact with the correct form, but doesn't work with new
contacts.
Today it didn't work with new contacts, but with some old ones, but
some old contacts crashed outlook..
Sometimes re-publishing the form in outlook helps, today deleting the
form from the folders formlist, corrected the problem.

Any ideas what this is all about?? I reinstalled outlook a couple of
weeks ago, now it happened again.

Have you applied SP1? Where is the form published? Are all users affected or just one?

FYI, there is a newsgroup specifically for Outlook forms issues "down the hall" at microsoft.public.outlook.program_forms or, via web interface, at http://www.microsoft.com/office/community/en-us/default.mspx?dg=microsoft.public
