I am trying to figure out how to format my phone numbers in my address book,
so I can read them easier.  Right now, the numbers look like 9995551212. I
found where I can add a 1+ 9995551212. but that doesn't help.  I wanted to
format the address book so all numbers look that same an are similiar to 1+
(999) 555-1212

You have to set up the Windows dialing properties in Control Panel.
