For the OAB it's Tools>E-mail Accounts>View or change existing directories

Where is your delivery location? Look at the bottom of Tools>E-mail
Accounts>View or change existing e-mail accounts>Next

Actually, the secret was contained in the text at the top of the dialog
you get when you do this... It says, 'To designate a Contacts folder
as an Outlook Address Book, go to the Properties dialog box for the
Contacts folder. On the Outlook Address Book tab, select the "Show
this folder asan e-mail Address Book" check box.'
After I did this, all the addresses were now showing up... I've
cleaned things up by deleting the other OABs from the profile as you
suggested as well.
