How do I show "Last Name" first rather than "First Name"  when printing my
address book?

There is no address book in Outlook. Your Contacts reside in your Contacts
Folder.
The best way to customize a printout of your Contacts data is to use a mail
merge:
See:
http://www.windowsitpro.com/Article/ArticleID/25809/25809.html
and
http://www.outlook-tips.net/howto/mailmerge.htm
use Mailmerge FROM Outlook to Word to create a printable list.
Or:
http://www.slipstick.com/contacts/print.htm
uses Access (linked to Outlook) as the reporting tool.
