Hi, I want to import all my ACT information to OUtlook. I have found a
program that does this to an extent. A lot of the field used in ACT are not
present in Outlook. For this reason I want to change the default outlook form
and see if I can then import from ACT. I have a couple of questions. 1. Has
anyone done this and had any success? 2. How can I change Outlook's default
contact form and have it stick? I have gone through the docs for this and it
doesn't seem to work. I must be doing something wrong. I get as far as
publishing the form but then if I close and reopen outlook the form is no
longer available. Any help would be greatly appreciated. Thanks Sameer.

You are probably missing the step where you publish the form with the Tools | Forms | Publish Form command.

However, Outlook doesn't support importing to a custom form. You'd have to write custom code or use a third-party application. See http://www.outlookcode.com/d/customimport.htm.
