Trying to import addresses into an Outlook contact list,
and have created a contact form that I want to use.  How
do you choose the contact form that is used when you
import?  Even though I have gone into properties for that
contact folder and changed the form it posts to, it still
uses the default contact form when I import?

This is normal behavior. Outlook doesn't support importing to a custom form. You'd have to write custom code or use a third-party application. See http://www.slipstick.com/dev/customimport.htm.
