using custom forms in Outlook 2003

I can't open my old contact items in my custom form? Also, my custom form was
not opened while i transfer my contact items to another computer ?

You exported and imported your contacts, didn't you? Forms are not included in such transfers. You will need to save the form as an .oft file, copy it to the other computer, and publish it with that computer's copy of Outlook before your contacts will be able to use it.
