How to set up a computer, phone and modem for auto phone dialling

I can understand the software support in Outllook 2003, but I need to know
what cable to plug into what to link my computer (with modem) to the phone so
that I can talk using the phone.

Outlook cannot dial a number and then turn the connection over to the phone
for voice.
