can I import from excel to outlook when I have a special template

I have a special template in my contacts.  It has different fields in
addition to the default ones.  When trying to import from excel to outlook, I
am only able to map the fields to the default template.  Is there a way to
transfer the data from excel to the specially created template.  When I
create a new folder in contacts to import to, I make sure that the properties
are correct and that the form I want to use is selected.

Outlook doesn't support importing to a custom form. You'd have to write custom code or use a third-party application. See http://www.outlookcode.com/d/customimport.htm.
