how do i set  up a form on excel to be used in contacts

I am trying to link an excel spreadsheet as a template for contacts. the
oulook help doesnt see mto cover this?

Create a couple of contacts in Outlook.  Export to Excel.  Copy the format
for your Excel contacts.
