default contact form open instead of custom contact form

I have created a custom contact form (2003) and saved it to "my documents" on
a friends computer (also 2003). When I browse to the custom form and double
click to open it, Outlook starts up the default contact form.

How can I get Outlook to open the custom form so I can publish it?

Choose Tools | Forms | Choose Form and browse to User Forms in File System to locate your form and open it.
