Custom Contacts Form

I recently created a custom form for displaying my Contacts (I created a new
page with some additional fields that I routinely enter information into). I
published the form to both the Personal Forms Library and the Contacts
folder. How do I actually get my contacts to defaultly use that new form
though so that when I open up current contacts or create new ones, my custom
form is used as opposed to the standard Outlook Contact form?
