Why does TO: field displays double name when creating a new email

My contact list is displayed as an address book. When I create a new email
and select TO: open contacts and select a name, the TO: field displays the
name twice.
One is in parentheses one is not. Contact display shows name and email
address.

do you have more than one email address or a fax number for the ones shown
twice?
