redesigning outlook contact forms

I am trying to redesign the outlook contact forms because my clients are
usually husbands and wives, usually with children. The current standard form
is designed to contain info on only 1 person, but I need to add business
address and phone numbers (both business and mobile) for both husband and
wife, as well as fields for birthdays for both and children fields
