When I use the Send To:  command in Word 2003, all I can see in my contacts
is the first tier of my contacts, not all the folders and people that are
actually in contacts. Is there a way to make it show all the contents of my
contacts list?

First, enable the folders as email address books in their properties.
Then select said folders from the dropdown when you are asked to choose a
recipient.
