Hello,
I am tring to import my contacts (dBase) into a custom Contact Form, but
every time I import and define feilds it put them into the standard Outlook
Contact Form.  Is their anyone who knows how to get around this?   Thanks for
your time and help.  Mark

Outlook doesn't support importing to a custom form. You'd have to write custom code or use a third-party application. See http://www.outlookcode.com/d/customimport.htm.
