New fields in my contacts form are not availabl. for mapping

When importing from Access into my Outlook my new fields (in my new form) are
not available for mapping.

Outlook doesn't support importing to a custom form or custom fields. You'd have to write custom code or use a third-party application. See http://www.outlookcode.com/d/customimport.htm.
