When I print my address book a contact's fields aren't showing up

I have designed a form with custom fields for a business contact. When I
print my address book, the custom fields don't show up, but everything
standard does.

What steps are you following to "print your address book"? Since Outlook has no separate address book, it's not at all clear what you might be doing.
