flag to follow up not working

I have created a custom form based upon a contact form, & now I cannot seem
to use a "flag to follow up" with it, can anyone help please

When I press the follow up flag button on my contact, then the box appeares
& allows me to fill in time details for follow up but wnen I press return
it does not show the gray follow up bar accross the top of the contact &
consequently does not pop up a reminder at the time it is supposed to.
outlook 2003 winxp 2.7gb proscessor igb ram
