Issues with Customizing Contact Form

I’ve customized a new form and the fields keep re-adjust from the way I
originally saved it (the length, placement, etc.).  How can I stop that from
happening?

Additionally, I have 214 contacts that I want to put into this new format.
Any suggestions on how to quickly and efficiently transfer the data vs
retyping all the info?

Thanks

Hi all gurus,
I've a problem with Outlook Contacts. When I choose an address from my
Contacts into the address field for a mail I'm composing, I'd like to have
the contacts shown, ordered by company, family name and then by first name.
How can I do that? Many thanks in advance
