How to get back the "NAME" button in CONTACT

When I first key in the new contacts, I am able to click on something like
"NAME" button. It enable me to key in the First Name, Middle Name and Last
Name. But now, I can't find the "NAME" button. I am not able to key in the
Middle Name for my contacts. Pls help! thanks alot!

You posted no information.
All versions of Outlook have a Full Name... button on the first page that
allow you to parse name elements individually.
State your version and the steps you used to lose the button.

Checked. I am using WinXP ver 5.1 SP2 and my outlook is 2003, ver11.
Even I try on restoring or reset the setting, I still can't get the "NAME"
button.
When I click on new contact or old contact, I only see "Frist Name" and
"Last Name" only.

Not possible unless you are using a custom form or are talking about
something other than the General Tab of the New Contact Form.
Again, the steps you used to lose the default form...?
