How can I get "Mr. and Mrs" titles in Outlook for printing

I need to print formal envelopes to "Mr. and Mrs." contacts.  I have created
contacts using the name and spouse fields, but I can't figure out how to add
Mr. and Mrs.  

I also can't figure out how to use the spouse field in a mail merge for
publisher.

Suggest posting Outlook version, data source and what Publisher has to do
with this.
Also post your merge field construction. "Mr. and Mrs." would of course be
text, not a merge field.
