I do I set up a Fax template to be used as an action from Outlook

I often use the Action New Letter to Contact from within an Outlook Contact.  
How can I use the same Action to create a fax cover sheet?  I assume I can
create a template, but am unsure how to do so.

You can, but the process has become so awkward and difficult that almost
everyone has abandoned "New Letter to Contact." Just create a Word document
as a template for a Fax Cover sheet and insert your recipient using the Word
Address Book (which links to your Outlook Contacts).
