What causes this and how can I prevent it from happening?

Hi,

Have you imported contacts into Outlook, or synchronized your contacts with a handheld device?  Either of these can cause duplicate contacts.

Clarify your post. No one can know what you mean by "contact list." That
term has no meaning in Outlook.
