Contact Default Form

How can I edit the contact default form?
I simply want to add one line for "Department" under the "Company" line.

I tried designing a new form by simply adding this field but the resulting
form is different.  For example in the default form I could click on for
example Business Phone box and a "Check Phone Number" box with Phone Number
details will open.  However when I use the designed form I cannot get this
option by clicking on the Phone box.

Tools | Forms | Design a Form, then see http://www.outlookcode.com/d/newdefaultform.htm

That's normal, expected behavior.

Thanks for the reply.
I read the article however I have not used Visual Basic and have no idea how
to proceed.

My knowledge is basic but I am willing to follow instructions.  Could you
give me some step by step instructions on how to add this script please.

There's no script to add. It is not possible to display the phone dialog programmatically.
