formatting contacts

I have been fighting with this issue (at least a year now) of getting a
single string of phone numbers on a new contact, instead of the (xxx)
xxx-xxxx format automatically.  Is there anything else to do in control panel
other than enter your location and area code?  I don't want to use
autodailer, I just want to use the contacts as my address book.  About a year
ago, I uninstalled and re-installed Outlook 2000 (from the office suite).  
Did not help.  Several months my computer crashed, and a reformatted the hard
drive and re-installed everything and the contacts formatted perfectly.  Now
a few days ago the auto formatting is gone again, back to a single string of
numbers.--

All you need to do is set a dialing location correctly for your OS.
Any idea why your OS might be losing track of yours?
