Importing a CSV file into Outlook - How to make name fields appear

Okay I exported my contacts from Earthlink into a .csv.  When I import them
into outlook nothing but their email addresses come in.  I can't seem to get
their names and address to be included.  I have mapped the fields, I have
converted to excel and mapped the fields.  What else???
Someone help me please!

An easy way to make sure you have the right format etc is to export a
contact from Outlook into Excel and then follow that format.
