State by which method you are attempting to insert the address.

In Word 2003, I'm using the address book icon from the toolbar. I select a
name from the Select Name box, then in the Advanced drop down box, under
properties, I can view the Contact box, then I click on "Full Name" which
brings up the "Check Name Box" and the name (Mr. John Doe, Jr.) is entered in
the proper fields but as stated in my question, only the first and last
names are inserted into my letters without the prefix (Mr.) or suffix (Jr.).

Modifying AddressLayout in Word 2002 and 2003 will apply only to addresses
inserted into a document, but not to Envelopes or Labels. The KB offers the
following kludgy workaround:
