Problem changing the view type in Contact

I go through all the steps to customize an address card,
then choose it - but when I go into add a new contact, the
basic info (not my custom info) is listed on the card.  

Example: I customize the card to include name, telephone,
address, medical diagnosis, current treatment.  That's all
I want on the card.  I choose it - but the address card
still says "name, job title, company, .... email address,
etc"  

Any help would be greatly appreciated.
