How can I delete a custom contact form?

I seem to have 3 copies of the same revised contact form, and I want to start
over. So I want to delete or at least rename the forms. Using Outlook 2003.
Can't actually find the folder I created to hold the form in the navigation
pane - used the tutorial for creating and distributing online forms on
Microsoft website. I can open the form through Tools > Forms and enter
information, but then the information doesn't show up in the phone list view.

We have no way of knowing where you published the form, assuming you did publish it. To delete published forms, you need to use Tools | Options | Other | Advanced Options | Custom Forms | Manage Forms

That seems to be the problem - where I published it. I created a folder at
the top level of my Inbox and published the form to it. However, when I
follow your instructions, I don't see that folder - it doesn't come up in the
list of Manage Forms and it also doesn't appear in the Navigation pane. I can
pull up the form from "Choose a Form" and add people to it, but then I can't
view them after I do that. I know the information is probably there - I just
can't find it.

I didn't know about the path you show below, though, so that was helpful in
itself and now I know how to delete folders - if they appear in the Manage
Forms list.

You would need to browse to that folder among the Outlook folders from the Manage Forms dialog. Alternatively, right-click the folder, bring up its Properties dialog and get to the Manage Forms dialog from there.
