importing csv file to contacts

I am trying to import a csv file of contacts into Outlook 2003.  The format
of the file is

"entry 1","entry 2",...,"entry n"

I go to import/export and select import from a file.  Then I select a Comma
Separated Values (Windows) file format.  Then I select the file to import..  
Then I select contacts as the folder to be imported to.

Then I get a screen that has "Import "filename" into folder Contacts".  
However the Map Custom Fields button is not highlighted and the Next button
is not highlighted.  At that point I can do nothing.  I check and the file
properties were archive so I uncecked the archive and tried again with the
same results.  Any suggestions on what to try next.

Ensure you have a Named Range in your .csv.  You may want to export a couple
of contacts from Outlook to a .csv format and compare the 2.

Thanks for the info.  Figured it out last night - needed to have the first
line have the data field definitions in it.  However, I am now having the
same problem with a calendar file.  This time I put the info in the first line
"Date","Begin Time","End Time","Test"
"date 1","begine time1","end time 1","text1"
etc.

Assume it's still something simple
