Setting a custom default form

I read the article Sue suggested, but do not know what to do.  I click
contacts then go to properties.  My custom form is in a box that says
associate this form with this folder, then there is a manage button.  The
manage dialogue box includes an install section and a save as section.  My
form is listed in the box on this page.  This is a different set-up as
compared to what is in the article.  Can you pls advise what I am doing wrong?

What makes you think you're doing something wrong? (especially when you don't give us a clue as to what article you're referring to or what version of Outlook you're using)

The default form for a folder is set on the folder's Properties dialog, on the General page, under "when posting to this folder, use."

Please use the suggestion feature only to suggest changes, not to request assistance with existing features.
