I would like to add a spouse birthday field in my contact record. Is there a
way to change the anniversary name to Spouse Birthday?  I have added custom
user fields under the tab All Fields but it does not automatically add the
date to the calendar.

Any suggestions?

Standard fields cannot be renamed, and you can't modify the Details page at all.

If you want to add appointments to the calendar, you need to tell Outlook that by writing the code behind the form to create them. Start with Application.CreateItem.
