Entry changed by antoher user

In Office Outlook 2003, we often start a journal entry for a contact, do
Ctrl-Save during typing, enter some more info, then see a warning dialog,
Title "M$ Office Outlook", Text "The item could not be saved because it has
been changed by antoher user or in another window.  Do you want to make a
copy in the default folder for the item?", buttons 'Yes', 'No'.  We answer
Yes in order to save new data entered.

We had not entered data in another window, and no other user accessed the
data.  The error is aggravating, and we then have to investigate and delete
the older incomplete journal entry.

How can we correct this strange Outlook behavior?
