Can I modify general tab input titles on Contact card file??

I have Outlook 2000.When you go to enter a new contact and
the general tab is the first one you see in the untitled
contact screen, Is there a way you can add to the standard
input area names?  In addition to Full Name, Email address,
etc. I want an input  category for Lead Origin Date.  I
looked in help, went to customizing view, adding fields,
etc, but it doesn't change this first and primary screen.
Please help ASAP!
