Outlook 2003 and Exchang 2003 environment here.  If you open a user from the
Address Book in Outlook there is a button to add that person to your
contacts. Where does this button come from and is there a way to remove or
disable this button?

That button is built into Outlook and cannot be disabled.

Thanks for the quick answer.  One more thing on this.  My partner also is on
Outlook 2003 the same as me, but he does not have this button.  Can it be
hidden?  He's not sure if it was ever there.

Maybe you're not looking at the same dialog? Or the same GAL entry?
