Changes to Conatcts in Outlook are not always saving

I work for a real estate company with a large database of addresses that we keep in Outlook and it is very important to our business to stay in contact with our clients.  Sometimes (not all, and this is what especially confuses me) when we update addresses or phone numbers for our contacts it does not save the new information, but reverts back to what was previously saved.  Can anyone give me an idea of why this may be happening and how we can make sure that it doesn't keep happening?  Any help would be much appreciated.
