Creation Date and Last Modified Date fields are not available in Outlook's
export wizard.

Have you tried avoiding the export Wizard and just selecting the table of
data you want in Contacts (use Phone List View and add all the fields first)
and just dragging to a spreadsheet? Then you can take the columns you want.
