Can I export my contact list into Excel and include the date created
information?

yes.  When you select the fields make sure to include it.

Creation Date and Last Modified Date fields are not available in Outlook's
export wizard.

Two options for you depending on your requirements:

Have you tried avoiding the export Wizard and just selecting the table of
data you want in Contacts (use Phone List View and add all the fields first)
and just dragging to a spreadsheet?  Then you can take the columns you want.
