1. Detect and repair is better than uninstall/reinstall
2. Are you using the same pst or did you create a new one? Published forms
reside in the pst, in the folder there were published in
3. Check the add-ins (tools, options, other) - is exchange extensions
installed?

Exchange Extensions was not on! Thank You!
Now the Forms Tab is back and I will try to load the custom form today and
let you know if it leads to any additional issues I can't resolve myself (so
please check back!), but before we close please tell me if I'm doing this
correctly:
1) To backup all contacts, email and forms, should I File>Import and Export>
Export to a File>Personal Folder File>...OR...File>Backup?
2) and...what's the best way to just backup a Custom Form by itself?

I've been going into Forms Manager>Save As...and then save the .fdm file to
a backup folder. Do I paste that file into Local Settings\Application
Data\Microsoft\Forms to re-load?
