I am trying to create labels in Word 2003 with contacts from Outlook 2003.  When I Choose Contacts Folder I get "Unable to obtain list of tables from the data source."  When I do it from Outlook my labels are blank.  Outlook is my default Email and all of my outlook folders are marked email address books.

Take a look at this MS KB article, it may help:

Ok I tried that.  I still get the same thing.  PS.  When I ran fixmapi, nothing really seemed to happen, is that normal?

Yes.
List the steps that produced your problem.

Ok.
Open Word.
New Doument
Tools->Letters and Mailing ->Mail Merge
Labels
Next: Starting Document
Change Document layout
Label options ((avery 5160)
Next:Select Recipients
Select From Outlook contacts
Choose Contacts Folder
Choose Profile (MS Exchange Settings - there is only one choice)
Ok
"Unable to obtain list of tables from the data source."

BTW.  Befroe I wrote this reply, I did the "Reinstall or Repair Office 2003".

No, I meant the configuration changes to your system that broke mail merge.
I assume it worked at some point.
Does your default Outlook profile have the Outlook Address Book configured
correctly to connect to your Contacts Folder? Are you running against an
Exchange Server?

I updated to Outlook 2003.

Sorry, I hit send before I answered all your questions.  Where do I configure the profile?  How do I determine if I am running against an Exchange Server?
Russ Valentine [MVP-Outlook] - 12 Jan 2004 11:38 GMT
You would know if you were running against Exchange Server. I assume you are
running Outlook on a standalone PC, not a corporate network.
Can you even open Outlook?
Follow the instructions here to configure your Outlook profile. They are the
same as for Outlook 2002.

Yes, I can open Outlook.  Once I get into Mail in the Control Panel, I click on Properties, what am I looking for in E-mail Accounts and/or Data Files?  In Email Accounts, under directory-> OUtlook Address Book->Change, I see all my folders.  In my Personal Address Book the path goes to Outlook\mailbox.PAB
Russ Valentine [MVP-Outlook] - 13 Jan 2004 03:18 GMT
You should not have a PAB in Outlook. Remove it. Use only the Outlook
Address Book.

Okay, How do I remove it?
samsmom - 13 Jan 2004 05:16 GMT
Thank you oh great one!!!!!  That seems to have worked (I just "removed" the Personal Address Book in the Adress books and directories)  However, I'm having another problem now.  I want to use the "Mailing Address"  However, that isn't a choice so I only get some addresses, depending on whether its a business or home address.  How do I get it to let me get the "mailing address"?
samsmom - 13 Jan 2004 05:36 GMT
Actually,  As I am looking through these addresses, there doesn't seem to be rhyme or reason as to whether an address is picked up or not.  I am using the "address block" so some addresses are picked up and some are not.
Russ Valentine [MVP-Outlook] - 13 Jan 2004 10:23 GMT
There are many complexities to inserting addresses into Word from Outlook.
The best method depends on your Outlook version and the version in which the
Contacts were originally created. Word has an entire newsgroup dedicated to
these questions. There are some general guidelines here as well:
