In all of my searching, I have been unable to discover how
to print only selected fields from a custom designed form.
Outlook wants to print all of the fields in the form
(template), and I want to only print selected fields. Does
anyone know how to do this, and can it be done at all?

See http://www.slipstick.com/dev/customprint.htm for various solutions to the limitation that Outlook forms don't support WYSIWYG printing.
