Importing a CSV file problems

When I attempt to import a CSV file into my contacts list
the Wizard performs all the steps up until you choose the
destination. When it reaches the Import a file window it
says the following actions will be performed but the Map
Custom Field box and the Next box are not highlighted to
have the option to continue.

Just highlight the destination and makesure the checkbox gets checked -
buttons will get enabled after that.
